aww man, any other week i'd be able to help you out... my office hours are slammed and i have to head to the airport in about 3 hours.
if you don't have your birth certificate or a passport i think (*) that you can get a statement of residency notarized by a public notary as long as you provide your driver's license and copy of paycheck and utility bills.
*not sure if this practice is still accepted with neighboring US countries but I believe it used to.
