see it's like this. I was born in San Francisco, right? And I wanna go to Mexico this summer so I need my birth certificate, right? And I can't find it anywhere in my house, right? And until July 1 someone who lives in or close to the city can walk in with the form (which I'll fax to you) and $15.00 (which I'll pay back to you) and get a notarized copy of the birth certificate, which you can then mail to me so I can voy a la playa. It SHOULD only take 15-30 minutes as per their website. (I'd mail it in myself but that'd take like three months.)

aww man, any other week i'd be able to help you out... my office hours are slammed and i have to head to the airport in about 3 hours.

however,

if you don't have your birth certificate or a passport i think (*) that you can get a statement of residency notarized by a public notary as long as you provide your driver's license and copy of paycheck and utility bills.

*not sure if this practice is still accepted with neighboring US countries but I believe it used to.

thanks for asking, g! old buddy old pal, but we don't leave until the middle of August. the great thing that is the California Dept. of Health really came thru for me, sent that b.certificate back in like 2 weeks instead of the 14 weeks that is the 'official line'. turns out that if you say 'this is for a passport' when you send it in they rush it. lucky also that I am the only person with my name that has ever been born in San Francisco, or probably California. (There is a kid in SD with my name, which is another reason not to self-Google.)
