When syndicating a show, the production company attempts to sell the show to one station in each television "market", or area, in the country and around the world. If successful, this can be lucrative; but the syndicator may only be able to sell the show to a fraction of the markets.
When a show is locally syndicated, it has been sold by a distributer to local stations/affiliates in as many markets as possible. In some cases, a show in local syndication may be seen in one state on one station but not in another state on another station (or, if there are several stations in one state, on one station and not another). Thus, a television program can be locally syndicated and seen by only a small portion of the television audience if only a few stations air it, or by a large portion of the television audience if many stations air it.

They had a funny bit on Sesame Street the other day where Dr. Feel was trying to do his thing and then Dr. Phil shows up and he's all liek "now I'm Dr. Phil this is my show", "no I'm dr. FEEL and this is my show." and they both got real angry.
