First, I have nothing to go on except news reports, which means I have no first hand knowledge at all.
Next, it isn't entirely inconceivable that Milosevic was murdered, so I can't rule it out instantly as I could, for example, a report in the World Weekly News that Hitler had a three-headed baby each of whose heads resembles one of the Three Stooges, and he is living in the Congo. Political murders happen. Three-headed babies do not.
Consequently, any conclusion I draw fits into the category of a belief, not a fact. All I can do is evaluate the probabilities, by whatever yardstick I can devise. Understanding this, I am willing to consider the possibility of murder...

One week ago the conspiracists would have you believe that Milosevic could not possibly receive a fair trial, that his captors and judges had fixed the verdict before they ever started, that Milosevic was totally in their power and the outcome was inevitable.
Now they would have you believe that Milosevic was a dire threat to them, that they were so afraid of letting him live one day longer that they risked murdering him. These two beliefs are so highly incompatible with one another that they have all the earmarks of willful self-delusion.
By contrast, the 'official story' at least makes some sort of sense in terms of motives, actions and outcomes. I have no choice but to believe it in preference to the other. (Not that I know the first thing about what really happened.)

Third possibility, that he committed suicide. His lawyer had suggested that the authorities were trying to poison him before his death. He knew that he wasn't going to walk free so by committing suicide he implicates the west in his 'murder', undermines future trials amongst his cronies, gets instant martyr status among Serbian nationalists and possibly helps ferment nationalist unrest in Serbia.
Alternatively he was an old man under a lot of stress and had a heart attack.
