I have summer job in an office where you can apply for city-owned flats, and I'm doing a rough English translation of the application form. So I thought I might ask you native speakers whether this sentence is grammatically correct:

I hereby warrant that all the information given above is correct, and, if necessary, I approve of it to be verified. If the information I’ve given in the application will change in the future, I will commit to correcting my application to correspond with the changes.

Also, is there an English word for the sort a support home that isn't for seniors nor the disabled? Is it just "sheltered home"?

Finally, is there a word for this kind of a house, which has two or more flats, but no closed staircase:

I would make the second sentence something like, "Should any information provided herein change, I will (promptly?) submit a corrected application (containing the new information? reflecting that/those change(s)?)."
