RFI: Movie on syndicated TV in the late 80's/ early 90's that scared the shit out of me

i'm trying to find the title of this movie that scared the shit out of me when i was a kid.

all i remember was that in a scene towards the end there was a girl trapped in an iron-maiden-type-thing that looked like a big dress or something with her head sticking out the top. she was on a big proscenium stage in some sort of theater in front of a rowdy crowd. there was this creepy dude who had kidnapped her walking around the stage with a crazy grin on his face and a big knife or something in his hand. the girl was screaming to be set free but the audience didn't know it was real and thought it was all some kind of act and they were all cheering for the creepy guy to stab her. the audience didn't know any better, they just thought it was an act. i can't remember if she lives or dies.

what is this fucking movie?

I don't know, but it's reminded me of a similar one where this woman's being raped by 100 or so men on a stage and the audience thinks it's a play. It might be a different bit of the same one.
