De-institutionalisation

Is it possible that if you're on the 'outside' too long, you'll never again be able to adjust to a controlled environment? How can people make the most of their stay in, say, a loony bin if there's nothing to help them in their escape from a hostile, confusing unstructured world?

I find the question itself a little confusing and unstructured, Dave. Do you mean 'outside' as 'outside the institution' or as 'inside the institution' (since loony bin residents are society's 'outsiders')? Is 'a controlled environment' the loony bin or normal life? And isn't a loony bin the perfect solution to those who want to escape from 'a hostile, confusing, unstructured world'?

This is already starting to sound like Wonko the Sane living Outside the Asylum.

You've got to get in to get out.
