my favorite instance of the ap being asleep at the switch was a headline about tyson gay advancing to the finals of the 100 meters in the olympic track qualifiers. apparently the ap uses some sort of auto-replace software to keep out potentially politically incorrect words. he was referred to in the headline and the accompanying story as "tyson homosexual".
no one caught it.
