heard this on the heston blumenthal/little chef show the other week and it reminded me of the kind of retarded management/marketing speak i used to hear on a daily basis from dinosaurs form the 80s in my old job. its pretty much a big brainstorm where you can hammer out any old idea. In reality it tends to mean 'we've listened to all the ideas/concepts/designs you've presented and we would rather you sacked them all off and do something completely different'

The (UK) Apprentice went to some design office or other for one of their tasks several series ago and the design office had a meeting room with clouds painted on the ceiling to encourage blue sky thinking.

That meaning strikes me as so weird and indirect -- I would have said with total confidence that "blue sky" thinking must indicate an extreme level of optimism. Y'know, as in "Blue Skies, smiling at me / Nothing but blue skies do I see."
