The 'Rock Bottom' / 'Octopus' double a-side 12" by King Krule is surely near the top of my list.

3. "Wrote for luck" Happy Mondays
Wasn't it only the DJ copy promo 12" released a year later that had the Vince Clarke/Oakenfold mixes on?

Um, I think that was the WFL normal 12".
Any case, I'm about the original one.

1. Sensoria : Cabaret Voltiare.2. KISSPOWER : Age of Chance.3. Master and Servant (On-U Science Fiction Dance Hall Classic Version) : Depeche Mode.4. The Nine Lives of Dr Mabuse (13th Live mix) : Propaganda.5. Out of the Flesh (Mix 1) : CHAKK
No surprises to anyone i know, but these are all still classics in my world.also, each of these tracks was released in a standard 7" version, and the 12" version is very different, as opposedto me choosing a techno track that was only ever released on 12".

^mark e's approach is probably the best way to answer the Q although I haven't quite followed suit with my 5.
Dan Hartman - "Countdown / This Is It" b/w "Instant Replay" (Epic, Canada 1978 - this one)The Crusaders - "Street Life (Special U.S. Disco Mix)" b/w "The Hustler" (MCA, UK 1979)Blondie - "Rapture (Special Disco Mix)" b/w "Live It Up (Special Disco Mix)" (Chrysalis, UK 1981)Frankie Goes To Hollywood - "Two Tribes (Annihilation)" (ZTT, UK 1984)Bronski Beat - "Why? (Extended Mix)" b/w "Cadillac Car" (Forbidden Fruit, UK 1984)
EPs released on 12" now, that would be a completely different looking Top 5.
