the brain is insensate, so it's never going to be itchy.

hydrocephaly (the accumulation of cerebrospinal fluid in the cerebral ventricles) can apply enough pressure to the not-yet-fused skull bones of an infant to force them apart, giving the characteristic "swollen-head" appearance. this doesn't happen in adults, though. the brain itself isn't especially swollen in either case (though there is some edema in the brain tissue at the edges of the ventricles).

embolic disease in the brain can produce an ischemic stroke in areas of the brain deprived of sufficient blood flow for too long. the death of those brain cells is followed by a tissue reaction called necrosis which can produce a great deal of swelling, if the damaged area is large enough. the brain is snugly enclosed in a hard case (the skull), and there's little room for it to expand. so acute swelling (for any reason, most commonly trauma and stroke) forces the brain to compress in on itself. this becomes life-threatening if the pressure is great enough to force parts of the cerebral hemispheres down through the membranes in the skull that usually support them, compressing the respiratory centres in the brainstem. this is the common mechanism of death for many types of intracerebral disease, including cancer and ruptured aneurysms.
