Is Kool Keith freestyling, in general.
Otherwise, I'd have to nominate the guest on the fifth track of Casual's - Fear Itself (I think, it's on the first side and it sounded pretty distinctive at the time.) I haven't heard this in over six years, my only copy is from memory. He sounds like he's waking up infront of the mic and leaving the studio.

I saw him. The three words of the night were "reciprocity", "Juice", and "pacbell" or something. He's blamed a lot for being gimmicky and his rapper-impersonation-schtick isn't all that impressive either. Dose-1 (of clouddead) is probably one of the more entertaining freestyles I've seen. "Seen" being key as his pelvis motions and violent trembling have to be witnessed for full effect.

Kool Keith, freestylin', is damned-near untouchable. The majority of his recorded output has been freestyled.

Method Man and Killah Priest are both amazing freestylers; Meth in particular is one of the most natural off-the-top-ers around.

Lateef and Lyrics Born, now those dudes own some freestyle. Haters be damned.

ODB has that drunken-master thing down, where it's not necessarily all that spectacular technically, but damn if his delivery ain't impressive as fuck.

I've yet to hear an Eminem freestyle I was much impressed by, but then I'm not all that impressed by his written shit either. But it seems like everyone else is. I just don't get it.

Dice Raw, now that dude has a vicious freestyle style. Any track he guests on, he sets it on fire.

Shit, Black Thought and Malik B. (I miss the M. Illah) from The Roots are some mighty impressive freestylers too.
