Severed Heads 12" - Dead Eyes..signed?

much to my surprise i was able to find a copy of severed heads 12" - dead eyes opened. while putting on the record, a nettwerk insert fell out of the record that had some writing scrawled on it.
it says "please send money to____" and then it's signed by tom ellard. has anyone else encountered this? did he write this on more records? i guess its probably just a pisstake.

That's right. Tom Ellard doesn't usually sign CDs etc. but leaves it to Ethel to do. Ethel is 102 years old and likes knitting.
