snrub it's not a fee, it's registering personal details of musicians and what type of music you'll be performing. so, if you put down "bashment" the police will turn up. if you put down "bluegrass" i imagine they wouldn't. this is completely... how you say... insane???

Because a great deal of the stuff in that application form is already covered in application forms for licensing of music - and the individual stuff is almost all stuff that ends up in the contract anyway if you're playing a proper venue.
None of this is particularly new. It's just not generally in one place like this - which makes me think this is for one-offs.

If this form allows you to have occasional one off live music events without adding live music to an exisiting license then this could be a good thing.
