There's a great (as in celebrated classic) Batman story, I think by O'Neil & Adams (or maybe Englehart & Rogers), where the Joker poisons Batman with a slow acting laughabilly gas and (no, positive it's Adams) it's perfectly scary when Batman laughs and grins. Not quite as demonic as that Detective Comics #600 Keith Giffen pin-up, but satisfyingly disturbing.
