Teeny would speak the line - while his midget minions would fidget nervously, knowing that whatever answer they gave to this obviously rhetorical question (which is uttered by Teeny eyeball-to-eyeball, lapel-in-fist, with one of the most nervous and incompetent of the mini-gangsters), it would involve the death of the least brave and clever of them, by either the rival minimobsters, or at the hands of Teeny himself.

Also, later on in the movie, Teeny should ask exactly the same question to a seven foot tall prostitute.

..and, very near the end of the movie, during a dark hour where all his midget cohorts and his 7 foot tall grunt have been killed, and it's raining, and he's alone in an alley, through tears, surveying the damaged wreckage of his gangster life, he asks himslef this question once again.
